What is ICSI?

Intracytoplasmic Sperm Injection (ICSI) is a specialized fertility treatment where a single sperm cell is injected directly into an egg, ensuring fertilization. Unlike traditional In Vitro Fertilization (IVF), where fertilization occurs naturally in a dish, ICSI involves an embryologist carefully selecting the best sperm and injecting it into the egg. This method is particularly beneficial for addressing male infertility or situations where conventional IVF methods may not succeed.

ICSI is typically recommended for:

  1. Low sperm count or poor sperm motility
  2. Abnormal sperm morphology
  3. Previous failed IVF cycles
  4. Challenges in fertilizing eggs during earlier IVF attempts

How Does ICSI Work?

The ICSI process involves several key steps:

  1. Ovarian Stimulation

Medications are administered to stimulate the woman's ovaries, promoting the production of multiple mature eggs.

  2. Egg Retrieval

After about 10–12 days of stimulation, eggs are retrieved from the ovaries using a needle under ultrasound guidance. The eggs are placed in nutrient-rich culture media for further processing.

  3. Sperm Retrieval and Preparation

Sperm is collected from the male partner or a donor, washed, and prepared in the laboratory for injection.

  4. Intracytoplasmic Sperm Injection

A single healthy sperm cell is carefully injected directly into an egg using a thin glass needle. The fertilized egg (embryo) is then monitored for several days before transfer into the uterus.

Benefits of ICSI

ICSI offers numerous advantages, including:

  • High success rate: Approximately 70-80% of eggs fertilized in most cases.
  • Ideal for male infertility: Provides solutions for low sperm count, poor motility, or abnormal morphology.
  • Addresses previous IVF failures: Useful for couples unsuccessful in past IVF attempts.
  • Reduced risk of genetic abnormalities: Helps minimize genetic issues in embryos.
  • Flexible options with donor sperm: Facilitates the use of donor sperm or banked embryos.

Eligibility for ICSI

Eligibility is determined through a comprehensive evaluation of both partners. During the initial consultation, the IVF specialist will conduct a thorough medical examination and recommend the most suitable treatment plan tailored to your needs.

Risks of ICSI Treatment

Like any medical procedure, ICSI carries certain risks and considerations. Potential risks include:

  • Multiple Pregnancy: Increased chance of twins or more, leading to higher risks for the mother and babies.
  • Ovarian Hyperstimulation Syndrome (OHSS): A possible reaction to fertility medications causing swollen and painful ovaries.
  • Ectopic Pregnancy: A slight increase in the risk of the fertilized embryo implanting outside the uterus.
  • Birth Defects: Some studies suggest a minor increased risk of birth defects, though the absolute risk remains low.
  • Genetic Abnormalities: Potential risk of passing on existing genetic issues from sperm or eggs.
  • Failed Fertilization: Not all eggs may fertilize successfully despite careful procedure.
  • Emotional Impact: The treatment process can be emotionally challenging, causing stress and anxiety.
  • Financial Burden: Costs can be significant, with no guaranteed outcomes, adding to the stress of treatment.

It’s crucial to understand that the likelihood of these risks varies by individual circumstances. Fertility specialists will thoroughly evaluate each couple's medical history before recommending ICSI or any other treatments.
